If an NDT was completed the previous quarter, and therefore is not due this quarter, the code 999 should be entered in the date field (NDTDATE) If an NDT assessment is due but cannot be completed the code 997 should be used in the date field. In either case 999 should be added to the score fields. You can use the notes field to provide an explanation.

Yes, this is acceptable. Please complete the NDT assessment based on circumstances at the point at which a service user engages with the project, not prior to this.
We are only asking projects to collect information using the NDT Assessment to provide us with a measure of individual need and how this changes over time. We are not asking projects to use the NDT assessment as a tool for determining access to services. If projects wish to use the tool in this way, that is fine.

The NDT Assessment is done by the project worker/key worker/navigator, without the service user being involved (unlike the Outcome Stars). It is in projects’ interest to complete the assessment as soon as possible after the start of engagement as this will enable us to capture all of the impact that projects have on an individual. We are realistic that it may not be possible on ‘day one’ but it should be done at the start.
