In the Outcome Star date field (OSDATE) enter 998 if a beneficiary has been with a project less than 2 months. Enter 997 in the date field for any other reason why an Outcomes Star has not been completed when one is due. You can use the notes field to provide an explanation. The Outcome Star score fields should be completed as 999.

The Stars should be completed every six months but the data is provided to the national evaluation team quarterly. If an Outcome Star is not due to be completed for a service user in a particular quarter, enter the code ‘999’ in the relevant fields in the data return.
